Student Registration Form
for Faith Formation and Youth Ministry

Please fill out the following form to register. Families must be registered parishioners. If not, please provide a letter of permission from your local parish.

Note: By clicking submit, I commit to attending Saturday vigil / Sunday Mass, holy days of obligation, and attend mandatory gatherings and retreat(s), as part of fulfilling the requirements of Diocesan expectations for curriculum, (and complete volunteer work for the sacrament of Confirmation).

Teens only: I give the MInistry permission to email and text my teen regarding church events and or any information pertaining to the Confirnation program.

Please pay via check (Attn: Mission San Diego, Memo: Faith Formation) or cash in an envelope (Attn: Mission San Diego, Memo: Faith Formation). Envelopes may be returned to the parish by way of the usual collection baskets, the Visitor Center and Gift Shop, or the US Mail.

Please fill out a release form for each child along with a registration form (see below).

All families must be registered at Mission Basilica San Diego de Alcalá. Otherwise, they must provide a letter from their pastor at their local church that gives them permission to have their child receive faith formation at Mission San Diego de Alcalá.
